CHPP 2025: Final Report Workshop

This workshop will be held virtually and is required for all current re-grantees.

Please RSVP by 3pm on Mon., 11/10, via email.

Final report forms are found on our website.

• Final Report: CHPP-FR 25
• Final Excel: CHPP-EX 25

Please email all final reports (in one PDF packet) after December 1 and no later than 3pm on Friday, December 12, 2025. 

• Updated narrative (include additional funds, changes in project, etc.)
• Financial reports (Income, expenses)
• Visitation numbers
• Required documents (see final report)

Organizations that do not submit their final reports by the deadline will delay release of final funds for FY 2025 and the application process for FY 2026.
